Step 1:
Schedule an Intake Interview
Please contact one of our monitors as soon as possible in order to schedule an intake interview. The interview must be conducted in-person with each parent individually before your first visit. During the interview, we will discuss the following:
- Set up a visitation schedule
- Review the visitation guidelines set forth by your court order
- Complete the EZ Supervised Visitation Service Agreement
- Review the fees and method of payment
- Safety and security measures
- Program conditions and guidelines
- Hours of operation
- Reasons for interrupting or ending a visit
- Termination process
- Help you plan your first visit

Before your first visit:
Under the State of California law, the monitor must complete form H272 in order to inform the court that he or she is the current monitor of your case. The form requests the monitor to confirm that he or she has fulfilled all requirements defined under Section 1166 of the California Penal Code. The form also requires the monitor to report any suspected incidence of child abuse within 36 hours of receiving the information of the incident. A copy of the form will be sent to all parties involved including the custodial and non-custodial parents, attorneys, and court.
Step 3:
During the visit:
A record for each case will be maintained, including but not limited to the following:
- A written record of each contact and visit including the date, time, duration of the contact or visit, and each activity performed
- All parent-child contact
- Observations of child and non-custodial parent's physical and mental state
- Mood of the parent/minor
- Any reminders of the guidelines from the monitor to the non-custodial parent
- How the visit ends
- Whether or not the visit is terminated and details of the termination
- Drop-off/Pickup details by both parents

Before your intake interview:
- You must fax, mail, or email, a copy of: a.) the court order, b.) photo of the child, c.) your driver's license or state I.D, d.) valid car insurance if transporting child, e.) any restraining orders or no contact orders.
- We recommend creating a list of questions you have for your monitor and make sure you understand what services will be provided and what is expected of you.
- You should also consider whether or not photographs can be taken and gift exchanges are allowed during the visit. (This is not allowed is sexual abuse cases, per California Standards 5.20.)
